Please complete this claim form to request reimbursement for your school's participation in Ride Illinois’ BikeSafetyQuiz Mini-Grant program. After we processes your claim, a check will be issued to your school. The check will be issued within 30 business days.

Please use this form only once a semester, when you are done using the quiz for that semester. Note: Do not report numbers for students who have not yet taken the quiz.
